Overview

This course provides an overview on practicing nursing in the patient's home, including what home health covers and who qualifies, how to assess a home as a care environment, fall risk reduction, medication safety without a pharmacy down the corridor, infection prevention with the supplies actually present, skin integrity, oxygen and equipment safety, and the family caregiver as the person delivering most of the care between visits, using Centers for Medicare & Medicaid Services, Centers for Disease Control and Prevention and National Library of Medicine sources.

Objectives

Upon completion of this independent study, the learner will be able to:

  1. Describe what home care services cover, from personal care and household help to skilled health care delivered at home.
  2. State the Medicare definition of homebound and the part-time or intermittent skilled service requirement.
  3. Distinguish services covered under home health from those that are excluded, including 24-hour care and homemaker services.
  4. Assess a home as a care environment, identifying the hazards named in federal fall-prevention guidance.
  5. Quantify fall risk for older adults using current national data and explain why most falls go unreported.
  6. Build a fall-reduction plan combining medication review, strength and balance exercise, vision checks and home modification.
  7. Reconcile medicines in a home where the list, the bottles and the patient's practice may all disagree.
  8. Apply hand hygiene correctly in a home without clinical sinks, including when alcohol-based sanitizer is appropriate.
  9. Identify catheter-related infection risk and recognise features suggesting infection beyond the bladder.
  10. Deliver pressure injury prevention using repositioning, skin care and pressure-relieving products.
  11. Teach home oxygen safety, including the fire risk and the prescription-only nature of oxygen therapy.
  12. Support and assess the family caregiver as a member of the care team rather than an unpaid extension of it.
